At Bradley Wellness, the focus is on building a long-term partnership that helps you feel vibrant, balanced, and in control of your health.

At the end of your visit, your provider will review your personalized treatment plan with you and send you home with a printed or digital handout outlining your next steps, recommendations, and follow-up plan.

Prior to your first visit:

To help us provide the best possible care, please complete your patient intake forms before your appointment. This allows your provider to review your health history and prepare for a more personalized visit.
Remember to bring your photo ID and insurance card with you to your appointment.

FAQs

  • We currently accept Blue Cross Blue Shield of Illinois PPO plans. We do not accept HMO or other insurance plans at this time. Patients without BCBS PPO coverage are welcome to be seen as self-pay patients.

  • Yes! We welcome self-pay patients. You can pay out of pocket to see a provider at Bradley Wellness.

    If blood work is done in-office through HealthLab, the lab will bill your insurance directly. Please bring your photo ID and insurance card to your appointment.

  • For your convenience, HealthLab by Northwestern Medicine provides laboratory services on-site at Bradley Wellness. HealthLab is in-network with most major insurance carriers.

    You are also welcome to use another lab of your choice—just let us know, and we’ll provide a lab order for you to take elsewhere

  • Yes! The “Be Well” IV infusion can be billed to your insurance. This simple hydration therapy uses electrolyte-balanced fluids to restore hydration, support energy, and promote recovery.

    All other specialty IV infusions are considered self-pay and are not billed to insurance.

  • Yes! We offer secure, HIPAA-compliant telehealth visits for your convenience. You can connect with your provider from the comfort of your home using a smartphone, tablet, or computer.

  • Yes! Many of our patients continue care with their existing specialists or primary care providers while working with us. We take a collaborative approach and are happy to coordinate with your other healthcare professionals to ensure you receive the most comprehensive, integrated care possible.

  • Yes. We ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. Cancellations made with less than 24 hours’ notice—or missed appointments—may be subject to a $100 fee. This policy allows us to reserve time for all patients who need care and helps keep our schedule running smoothly.
